The img file is a disk file that you can write directly to your sd card with the image writer software of your choosing.
The USB-SD.Creator.Linux-64bit is LibreELEC's own image writing application.
The first one is necessary, the second one is optional.
I'm not talking about add-ons I'm talking about straight up locally hosted video files.

Your comment makes out like I have every illegal add-on known to man.
Well, some people manage to get close to that number. Doing an initial search via the Kodi paste service, your log file lights up like a christmas tree. See the orange colored entries in the addons section of your log: hastebin . Those are the bad ones.

Math is perhaps not your best quality. I see at least NINE banned repositories, as well as at least SEVENTEEN plugins and scripts related to piracy.
Kodi and LibreELEC have gotten a bad enough reputation over the years because users think it's a good idea to illegally facilitate video piracy. If you, for whatever reason, are unwilling to provide a log file of a "clean" Kodi installation, then you are free to find your Kodi support away from the LibreELEC and Kodi forums,
How can I update from OpenElec ?
OpenELEC probably also has a smaller system boot partition than LibreELEC has. A fresh installation of the LE 9.x versions will solve that.
Whether you also do a full backup on OpenELEC first, and a restore to LibreELEC later, is up to you.
First, kindly remove ALL the banned add-on crap from your Kodi setup. I'd say that covers about 95% of your currently installed repositories.
We have the same policies as on the Kodi forum, and that means that any talk or usage of add-ons that facilitate video piracy is off-limits.
Only the LibreELEC installer overwrites a full disk, not an already installed OS with the Kodi program.

BTW: It's a bit annoying that this beta version with such an issue is offered and installed automatically when the options are checked in the LibreELEC configuration section, and after the reboot, users going this way will see nothing else than a black screen...
It's a beta. Accidents and errors can always happen. You should know that when using test versions. And yes, every bug is annoying, but that's why there are test versions.
A predetermined ETA for a patch is not usually given. All devs work in their own free time, whenever that is.
I had to file off a bit off my HDMI cable plugs to have a decent fit. Most micro HDMI plugs are still a bit clunky.
Is the setting "Disable other displays" enabled or disabled?
Cloning should be possible (both screens having an identical resolution), it's the only mode that Kodi supports.
Dual screens work on Raspbian Buster on a RPi4 here, so technically it's possible.
I have the same setup and I never get an automatic pop for cleaning any library. Did you enable Clean Library at startup, and/or perhaps install also other add-ons? Perhaps providing the kodi.log file via a paste website will give us some further insight.
I still can't see a graphics problem resulting into bad blocks on a storage problem. But anything is possible, I guess.
Those errors are typical for broken storage devices.
So... How are incorrect internal memory pointers & segmentation faults exactly causing broken (external) storage devices? It sounds like a big leap to me.
Perhaps LibreELEC Generic has a memory leak somehow. 
Install a clean & new setup of LibreELEC, install no add-ons, and let it idle for 3 days. Then check.
Are you perhaps using some kind of download manager? Which mirror(s) did you try?
The RPi2 image is by far the most downloaded image, so it would be odd that it would be corrupted (unless it happened very recently).
Kodi on the Nvidia Shield's Android runs fine for me.
No idea why you would want LibreELEC on it instead. 
Nope.
Outside access means opening ports on your router. Default web access is via port 80, but I'd advise to use a different port and remap that inside your router (if it's capable of doing that).
