The easiest way to "run" it is:
1) mkdir /storage/bin (move ncdu to this folder)
2) edit /storage/.profile and add PATH=/storage/bin:$PATH

Where are you running it from ? /storage ? It runs fine on ARM, X86 and VIM (aarch64) so it might be an O2 issue.
What's the output of file ncdu
"mc" is already available in the LE repository --> Program addons --> System tools.
You haven't specified what machine you are running - but there are statics available here.

When you say "missing" was it there in the past? I don't believe it was, as it's a fairly precise detail.
Yes it's possible to alter the skin, but it won't be done by us. I believe something similar was tried on the confluence skin, but it was particularly useful.
If you really need these details, then they are available via ssh and download bcmstat.sh it will give you everything you desire.

Running out of ideas but your dmesg log (at the end) showed a USB disconnect. Double check you cables and change if possible.
Try:
1) A fresh install without any addons/customisation (bar TVH)
2) Install the LE addon "system tools" and monitor the system with either nmon, htop to see I/O and memory usage.
3) Check free memory withfree -h
4) Check you USB ports, but adding a USB device and check dmesg for any disconnets
5) Check your power supply and change if possible. Also check output of vcgencmd get_throttled if NOT 0x0 then you have issues.
Try moving your dvb card to USB3 so that both use the same interface.
The OS has no idea what has priority on disk I/O. Well you could play around with lowering "nice" on the copy, or give TVH a higher priority but I think this won't help much.
Schedule the copy to run at a time TVH is not recording or just record direct to the USB.
Depending on your tuner, there may be USB conflicts.
Try stopping TVH first, then move the files.

On the RPi4 it is hoped in the future to be able to boot directly from USB rather than SD/USB. When this will happen, there is no timeline.
Q1 - Yes, USB is much faster than SD - (especially with USB3), so storage on an external USB/SSD is far better (a SSD is even better). Then there is little I/O on the SD card.
Q2 - I always use UUID as it's safer.
Q3 - I'm not aware of anything that would cause that.
