It's a bug in the settings add-on. The cache file is used, but it has to be activated by touch /storage/.cache/hostname.
Check those partitions for errors.

Sometimes a specific density is necessary to make a graphics file work. Take a working one, and edit it. So you'll keep the density.
For 4K, all HDMI cables should be conform to HDMI 2.0 or 2.1.
If that doesn't fix the issue, maybe your amp doesn't like 4K data rate.
